Deceased brought a full tub out of a level road on to a main haulage brow, dipping 1 in 6. The haulage was worked by over tub endless rope, and while deceased was placing his tub in position for attaching it to a rope, the lashing chain attached to two tubs, 26 yards up brow, broke. There was a trailer attached to these tubs, but they rode over it, ran down brow and struck the tub at which deceased was working and inflicted injuries from which be died on the 12th January, 1914.
