Deceased ran forward a loaded tub from the shaft to a mechanical side-tipping tippler he had some difficulty with the tippler and the operation was delayed and in the meantime another full tub moved forward down the slight incline from the shaft and after he had restarted the tippler by means of a lever he was crushed by it against the revolving tippler and tub. The Local Inspectors produced no written report at the inquest but one of them stated that they had examined the place and thought it an accident. Died 27th February, 1904. Deceased slipped on a sleeper about 2nd Oct and was never able to work afterwards. No inquest was held.
