A full tub ran amain in self-acting incline, left the rails and stopped 70 yards from the top. Three datallers were engaged putting right the tub. Deceased went from the top to about 30 yards from those men, whilst at that point he gave a signal by the bell wire, and soon after was found lying on his back between the rails, dead. Doctor proved he had been electrocuted. An armoured cable was on one side of the brow in close proximity to the signal wire. It was proved that the armouring of the cable was alive from leakage of current, probably at the gate end switch box. The armouring of the cable was not earthed by proper clamps, but by thin wire loosely twisted round the armouring of the cable. Also the earthing plate was not effective. Current 595 volts.
