Machinery – He was ratchetting in the bar of a coal cutting machine while in a kneeling position. Owing to the ratchet key slipping suddenly off the pin he overbalanced and fell on to the revolving bar. He received severe injuries to the right side and back of the upper part of the body and died almost immediately.
