When the descending cage left the surface the gate which closed the opening stuck and deceased allowed it to remain leaving the side unprotected when the ascending cage came to the surface he took off tile loaded tub, and in bringing forward the empty tub he pushed it into the open side and fell into the shaft with the tub.
