Deceased was standing on the front buffer of the first of a train of 16 empty wagons which was being pushed by a locomotive along the colliery sidings. The wagons failed to take the points and the first two wagons left the rails. Deceased was thrown from his position, and fell on toe rails. The first wagon passed over his legs and his left arm, and inflicted injuries from which he died on May 26th.
