Whilst ascending the shaft accompanying old pump set, the pipe caught, the engine continuing so strained the pulley the rope was liberated, 30 feet of slack rope went down the shaft. Deceased and his companion were jerked off, deceased falling into the water in the shaft, the companion fell on to a staging, having an arm and a leg broken.
