Deceased and the surveyor had been engaged levelling in a mid seam, and on arriving at the shaft the signal was given for them to he raised to the surface. When all was really they stepped on to the cage, and as soon as the cage began to ascend, deceased, who carried the levelling staff in one hand, apparently lost his balance, and before he could regain it, he fell off the cage into the shaft.
