Inspection made and inquest attended. Deceased was about putting the catch up for pulling the empties out of the cage at pit bottom when the thimble of the other cage which had ascending down the shaft upon him. A guide rod had been changed at the other shift and the block had only been secured by one nut on bolt instead of two as should have been done.
