Deceased was just about to unload a cage of empties which had readied the shaft bottom, when a tub, which had fallen out of the ascending cage, struck the cage and rebounded, fatally injuring him. The tub catch had been left open on the ascending cage, or at least the catch was open when the cage reached the surface.
