- Deceased, having landed a full journey at the pit bottom, had his body through the fly wheel of his engine and was oiling a bearing. In the meantime a shackler, having hitched the main rope to the outer end of an empty journey and the rope of a blocking engine to the inner end, gave a warning signal to deceased, and a signal to the blocking engine to pull in towards the faces. The result of the blocking engine pulling in was that the drum of the big engine was pet in motion, and deceased was crushed between the spokes of the fly wheel and the bedplate. He had omitted to threw his drum out of gear after landing the journey.
