He was having a set of 29 full tubs taken out of a landing when owing to four of the tubs being off the road a coupling between 8th and 9th tubs broke and the eight tubs in front were jerked forward with great violence and he was caught and killed by them. It was his duty to see all was in order before the signal was given to the hauling engineman to start his engine but he neglected to do so.
