- Deceased received an injury to his head while heading forward to lift the lever at the cage end to release the catches, presumably from a piece of coal falling from the cage, which had just ascended. The cages are double decked. The bottom deck, which drops on the keps, had been dealt with, and the top deck had been lowered to the keps level, when the onsetter heard a noise as of coal falling in the shaft. He stepped back a few yards, and at the same time warned deceased. Between the overhead protection and the top rim of the cage there was a space of 10.5 inches, and it was through this that the piece of coal passed.
