- At the commencement of his shift he was informed by a boy that a prop, which he had set two days before near a gate junction had been knocked out the previous day. Without examining the roof he commenced to re-set the prop, when a single piece of bind weighing 3 tons fell and killed him. The stone was noticed to be insecure two days before. The boy violated Regulation 7 in not informing an official that the prop had been knocked out by his tubs. Two props were displaced by the fall.
