Inspection made and inquest attended. He was attempting to couple a full tub attached to an endless rope to one of three tubs standing bottom of tunnel when he took more time then usual to couple the tubs which allowed the full tubs to be drawn on fatally crushing him against the tubs standing there. It was recommended in future to use a wooden block to prevent such another accident.
