- He was descending the No.4 upcast shaft in which there was only one cage to the Abdy Seam for the purpose of repairing a heading machine in that seam. At the time there were two colliers and two hurriers at work and one of the latter acted as hanger-on. The banksman allowed the deceased to get into the cage without sending a signal down to the Abdy seam and receiving a reply from the hanger-on and, as a consequence there was no hanger-on to receive him, and in getting out of the cage he by some means feel into some water at the bottom of the shaft, a distance of over 300 yards and was killed.
