- Deceased was in charge of a steam pump placed near the foot of an upset in the Red Mine Ironstone workings, Some datallers were about to take down some roof in the upset, and deceased was covering up the suction pipe of his pump to protect it from injury. A piece of roof dirt 7.5 ft. by 3.5 ft. at extremes and 6ins. in thickness, suddenly fell without warning and a portion of it struck him fracturing his thigh, and injuring him internally. He died seven days later from peritonitis caused by a ruptured liver. The upset was less than 6 ft. wide at the roof and was said to have been sufficiently timbered. It was examined by the fireman 15 minutes before the accident took place and appeared to be safe. There were no slips in the roof, which, however, may have been loosened by steam escaping from the pump.
