- Deceased was taker-off at the bottom of a jig in a level in the Moss Seam. He was sitting with his hack against the chock at one corner of the jig when a heavy fall of roof occurred in a cavity above the timbering along the level. A bar was broken by the fall and a large quantity of small debris came through the hole and buried deceased who was probably suffocated. The level was timbered with stout bars with laggings and runners above and partly stowed with dirt above the timber. It seems, however, that the space above was not sufficiently packed to prevent any further fall from dropping heavily on the supports. Further stowing with debris and timber above the bars in the cavity which extended to a height of about 7ft. into the roof might have prevented the occurrence.
