- Deceased having lifted the sump planks for the purpose of winding water, noticed a projecting strand on one of the locked coil guide ropes, and having failed to find the corresponding end, decided to go up the pit to consult the pitman. As he was going up the lower cage shoe must have caught under the other projecting end of the broken strand and caused extensive travelling, which impeded the speed of the cage and eventually lifting the guide, bringing the guide weights in contact with a staging in the sump. This caused a tilting of the cage which threw deceased into the pit.
