Inspection made and inquest attended. Deceased when attempting to unlash the chain from the empty tubs got fatally crushed by the tubs against a low bar. There was a low portion of the haulage read about four yards in length. It was recommended at the inquiry that the roadway should be enlarged as early as possible.
