Five empty wagons were being pushed up by a locomotive to the empty sidings. The duty of the deceased was to get into the first wagon, and when all was clear to give a signal to the locomotive driver to start. On this occasion be appears to have got on to the buffer of the first wagon and given the signal, ad while the wagons were moving to have slipped off and been run over.
