Deceased attempted to take an empty tub off the cage at the laigh scaffold before it had rested on the shuts the result was that one end of the tub was on the cage and the other end on the ground. The engine keeper got the order to lower but instead he raised the cage and the tub fell into the shaft and he with it falling a distance of 60 fathoms.
