- Bentley bored a shot hole in the ripping, 1ft thick, in a dip 3 yards wide in the Black Band Ironstone (inclination 20 degrees). He charged the hole, and then knocked out a temporary prop which had been set near the middle of the dip just beyond the edge of the rip. While bending to pick up the prop, the ripping fell over an area of 5ft. by, 4ft. from two slips, one running across the dip near the face, and the other running parallel to the side of the dip. The lower end of the fall struck him in the head killing him instantly.
