- Deceased was the regular cleaner employed in the engine house. After the pit had been idle for about 10 minutes, waiting for coal deceased got inside the substantial iron, fence and went to the left hand crank-pit, out of the sight of the engine driver, either for the purpose of screwing down a grease pot or picking something out of the crank pit. While he was in there a signal to wind came to the engineman, who did not know where the cleaner was. He started to wind and the crank in its first revolution struck deceased on the head and caused injuries from which he died in a few minutes. The electric bell on which the signals are made is right in front of the driver and can usually be heard in any part of the engine house.
