Deceased was assisting the onsetter in pushing tubs from the shaft round into the empty tub pass-bye. When the cage appeared he took hold of a tub in the bottom deck with the intention of doing the assistant bankman's work. The cage went on and he was pulled down, and his head and shoulders were crushed between the middle hoop of the cage, and the staging at the inset. He was dead when released.
