- Deceased was assisting a miner to clear debris from the right side of a horse road to make room for a new pass-bye, and had been so engaged for the preceding ten days. The pack on left-hand side was not made up to the roof, and the total width of unsupported roof was about 35ft. A slab of roof measuring 22ft. by 15ft. fell suddenly upon the pack, and the outer portion of it measuring 12ft. by 3ft. by 10ins. broke off, and fell upon deceased, killing him instantly. The miner, who had worked in this mine for 19 years, bad tested the roof every day, and had carefully sounded it with a hammer five hours before the accident took place & considered it to be perfectly safe. The Manager and Overman visited the place four hours prior to the accident and failed to observe any sign of danger. Although the area unsupported was excessive, the roof at this particular point had been exposed for nine years without repair, and was believed to be strong. No slip or break was visible, but it would clearly have been more prudent and would probably have prevented the accident if the pack had been completed up to the roof.
