A steam crane which he was driving, turned over when lifting a block of granite about 4 to 6 tons in weight (working load being up to 10 tons), in consequence of the wooden cill or anchor to which a stay was fastened having become decayed, and the bolt connecting them drawing through it. Deceased was pinned by the crane in a manner which prevented his release for some 20 minutes, in which period he received burns which resulted in death when released. The cill, which was loaded with granite blocks, had not been examined for an unknown period, the present Manager having omitted to have it done when the crane was under repair about 14 or 15 months ago when he took charge.
