He was coupling empties as they came off the cage at the bottom of the shaft, and appears to have pushed one towards the shaft, a foot or so, for the purpose of coupling up the shackle; the cage came down as he was doing this and caught one end of the tram causing the other to fly up and strike him in the chest, causing fatal injuries.
