Whilst descending the shaft alone in a cage his foot caught, in some way or other on the end piece of the shaft timbers which, in all probability caused him to extend his leg outwards beyond the edge of the cage. Before he could recover himself the top loop of the cage caught him across the hip, and the descent of the cage was arrested by his leg being wedged between the cage hoop and an end piece of timber 6 feet below the one on which his foot caught. His injuries were such that he died on the following day. If he had stood in the centre of the cage, the accident would have been avoided.
