Deceased was at work in the well of a slack elevator 25 feet below the ground level. At the same time two mechanics were engaged adjusting a driving pulley on its shaft 10 feet above the ground level. They had placed a cast-iron torch lamp on top of a pedestal, and one of them in turning round accidentally upset the lamp, which fell through a small opening in the floor and struck the deceased on the head, causing severe injury, from the effects of which he died on the same day.
