He was crushed between full tubs whilst taking a loose clip off one of them. This was not his work, which was to throw clips into empty tubs as they gravitated past him. He probably saw the loose clip on one of the tubs of a full set of 15 standing on the bank head opposite him, and whilst lifting it off was crushed between them as they were pushed forward from behind.
