Had helped to place a tub under a coal spout, sat on the side of tub whilst it filled, in getting down went near a vertical shaft 2.5 inches diameter revolving 28 to 30 times per minute 5 inches from a wall, jacket caught around the shaft squeezing him against the wall. Had been in operation 60 years without protection. Now securely fenced.
