Deceased, along with another sinker and a chargeman, was on an open, hanging and moveable scaffold preparing to fix wall plates to the timber of the shaft. The scaffold was being lowered with the men on it, when either the muzzle pin at the junction of the chains, or a corner of the scaffold caught the timber of the shaft and then jerked free, causing the scaffold to oscillate violently. Deceased had not hold of the chains, he was thrown off and fell to the shaft bottom, and was so severely injured that he died ten minutes later.
