- Inspection made and inquest attended. While he was attempting to push a tub further in the cage, with his foot, and pulling down the bow, around the end of tub; he appears to have got his foot fast, after telling his mate to signal the winder to start. He was carried up about six yards, to the shelter covering, where he fell back, down the shaft into the sump. He died from pneumonia, accelerated by the accident, April 17th, 1906.
