- Crushed by the cage at the four-feet mouthing in upcast shaft, had signalled himself to be lowered down the shaft, whilst entering the cage it descended, crushing him whilst half way in. Either the engine-man did not allow sufficient time for deceased to get into the cage or deceased unduly delayed entering after giving the signal. Probably the former occurred for the engineman would not know the man signalling had to afterwards get into the cage.
