Deceased had hung on a set of four loaded tubs on an engine dip having a gradient of 1 in 3 and notwithstanding repeated warnings, was following them up to shift the points for the next empty set. A coupling broke at a very defective weld, and three tubs ran back and crushed him against a junction post, killing him almost instantly.
