- Deceased having been sent with a message to the examiner and shotfirer was passing up through this working place just as a shot was fired. He was not seen by the men sent to give warning of the shot and was killed. This was due to inadequate warning proper means not having been taken to block all the entrances and see that all was clear before firing the shot. The examiner or shotlighter should have given the matter more care. It shows laxity on his part as an official. No doubt many other accidents occur through the same cause but cannot be so definitely fixed as in this case.
