- Fell 200 yards in shaft. No eye-witness. Engineman verbally told to raise the rope with four chains and hooks from surface lauding to pitbrow (16 feet) there the chains were to be hooked to a square tub to lower sand down the pit; the signal to raise was given by deceased, the rope raised and no further signals being received by the engineman went and found deceased missing the body found down the shaft. There were footmarks on the boards at the top landing and it is thought deceased had recklessly taken hold of the rope chains and hung on to be raised to the top landing; in a attempting to swing over the boards he failed and fell from the chains.
