This accident was due to a breach of Special Rule 91. Deceased was on his way along the main endless rope haulage road to the shaft at the end of his shift, when in passing a branch road near a steep part of the main road a journey of full tubs was being attached to the rope. A few yards beyond, deceased got on the front of these tubs, and had not ridden more than 5 yards when he was caught between the tubs and a low girder and so severely crushed by it. He died the following day.
