- Fall of roof on a heading. As he was walking along a heading road, accompanied by an Inspector of Mines and the Assistant Manager, a fall of roof occurred which knocked the Assistant Manager unconscious. As he went back to help another fall came down causing a compound fracture of the left leg. After amputation, he died on 22nd May, 1914.
