- Deceased, without authority, scotched back the props or keps at the pit bottom and passed through the top deck of the cage to the opposite side of the pit to assist the cager to bring a couple of fell tubs to the pit. The cager placed one of these tubs on the top deck and signalled to the engineman, and the cage was raised for the purpose of loading the bottom deck, when deceased, apparently forgetting that he had put the props or keps out of action, attempted to push the empty tub out of the cage before it had come to rest. He was caught and crushed between the cage, whilst it was being lowered, and the edge of the landing and severely injured. He died the same day.
