- Deceased was riding on the shackle connecting two tram of a journey composed of 20 full and seven empty trams which were being drawn up an engine plane, rising 1 in 6. When part way up, a shackle pin gave and 13 full and seven empty trams ran back on to the barhook. A pair of wheels got detached, shot to one side and smashed deceased's leg. The condition of the shackle clearly showed that the pin had not been put properly in place. Riding on this road had been prohibited on account of a portion of it having recently become low through heavy squeeze. Deceased was therefore illegally riding. He died on the following day.
