Inspection made and inquest attended. The jigger attached the jig rope to the full tub but omitted looking to see if the brake was tight. The full tub ran wild down the jig crushing the deceased at the bottom of jig. At the inquiry it was recommended that the stop-block should be placed on top of the jig and the signalling arrangements improved.
