Deceased was employed to keep the Main Ingoing eye in order and to see that nothing stopped the rakes of full or empty tubs. A rake had passed up and was just at the top when a coupling hook straightened out and four tubs came back 66 fathoms at 1 in 2 inclination and struck him. He was so severely injured that he died 10 hours later.
