Deceased and another reached the shaft bottom just as the lower deck of a two-decked cage had been placed in position, a man being already on the upper deck. The onsetter shouted to ask whether they wished to ride, and one of the men replied that he would wait for a mate. The onsetter rapped the cage away, and simultaneously the deceased rushed forward to enter it. The cage was raised as he was doing so, and he was caught against the receiving rod and decapitated.
