- Deceased was assisting three men to take off the balance weights in order to tighten the haulage rope, and when in the act of replacing them the rope was suddenly jerked, which caused the hanger and one weight to jerk up, and he was caught by the head and killed. It was afterwards found that the cause of the jerk was three men improperly lowering some empty tubs down a drift, dipping 4.5 inches to the yard, by using the clip as the brake on the haulage rope and when the tubs got about half way the clip met an inequality in the rope which caused the rope to jerk.
