Fall of roof at face of Barry work in Ras Las Seam in consequence of not being properly secured. At least two more props should have been set in the place even the maximum distance for roof supports under Special Rule 2 was exceeded. Deceased and Henry Thomas, the collier in charge of the Barry work, both seem responsible for the accident but proceedings do not appear to have been taken against Thomas which I think should have been done. Deceased paid the penalty for his neglect with his life.
