Deceased and two others were engaged in ripping a haulage road in the Ash seam. A piece of side which was overhanging had been spragged before, commencing to rip. After a part of the ripping was completed, this piece was examined and considered so safe that arrangements were made for boring in it to blast it down. As deceased was walking past, it suddenly fell without the slightest warning and struck him on the head, causing instant death.
