The dip cage was being loaded and bottomer pushed forward a tub to complete the load on deck, when it ran too far to the dip side, and striking a "leg" threw it out and caused girder to fall, releasing some debris which fell on deceased. He attempted to keep the tub off the "leg" but failed.
