Fall was being cleaned by man filling into tram which was being picked up by journey by direct haulage - an empty had been tumbled to be shunted down to fall - he was standing by manhole outbye end of tumbled tram - journey went out when tram caught tumbled tram and crushed him - pelvis fractured.
